HPRP Medium-Term Assistance

* AOG HPP Medium-Term Rental Assistance (4-18 Months): The AOG HPP Medium-Term Rental Assistance Program is an income-based program, thus the participants must be certified to participate in the proram. To do so, the participant will be required to complete an Income Verification Form for all family members over the age of 18 residing or will be residing in the household during the 4-18 month program. Complete an Income Verification Form for certification and complete all required documents before final approval.

Ø
Obtain approval of funds from an AOG HPP representative and present this amount to the rental property manager.

Ø
Speak to a financial resource specialist who will ensure that the participant can indeed afford the living space they are seeking to occupy.

Ø

Participate in Case Management/Wrap Around Services for the duration they are in the program.

Ø
Must re-certify every three (3) months for this program. Please note that this re-certification process will not disqualify you from participating in the program, but will help determine your cost of rent and utilities.
